Here's something worth knowing if you're taking Moduretic (the amiloride and hydrochlorothiazide combo). Coconut water may gently nudge your blood pressure down a little on its own, and since this medicine is already lowering it, the two together could add up. In theory that might leave you feeling lightheaded, dizzy, or shaky when you stand up. To be clear, this is a theoretical concern based on how things work, not strong proof, so an occasional glass with normal hydration is unlikely to cause trouble for most folks.

The bigger thing on my radar is the amiloride part. Amiloride keeps potassium in your body, and coconut water is naturally high in potassium. Loading up on concentrated coconut water could push your potassium too high, which matters more if your kidneys aren't perfect. Please run your usual intake past your pharmacist or doctor before making it a daily habit.

What Is Amiloride and Hydrochlorothiazide Used For?

Brand names: Moduretic, Amilzide, Moduret 25, Amil-Co

Amiloride hydrochloride and hydrochlorothiazide tablets are used for patients with high blood pressure (hypertension) or congestive heart failure who tend to develop low potassium (hypokalemia) when taking a thiazide or other diuretic alone — or for whom keeping potassium levels in a normal range is especially important, such as people taking digoxin or those with significant heart rhythm problems.

The tablets may be used on their own or alongside other blood pressure medicines, such as methyldopa or beta blockers. This combination product is not intended as the first-choice treatment for swelling (edema) or high blood pressure unless low potassium is a specific concern.
