This combo pill pairs a water pill (hydrochlorothiazide) with a beta blocker (pindolol), both of which lower your blood pressure. Coconut water has its own mild blood-pressure-lowering effect, so drinking a lot of it could nudge your pressure down a bit more than the medicine alone. In theory that might leave you feeling lightheaded, dizzy, or faint, especially when you stand up quickly.

I'll be honest, this concern is based mostly on how these things work rather than strong proof, so an occasional glass usually isn't a big deal. One other point: hydrochlorothiazide can lower potassium, while coconut water adds some back, which is generally fine but worth watching if you have kidney trouble. Check with your pharmacist or doctor before making it a daily habit.
