Corydalis Yanhusuo and Guaifenesin

Here is the interaction we found between these two, with the evidence behind it.

Moderate

DEXTROMETHORPHAN (Robitussin DM, others)

Theoretically, Corydalis yanhusuo may increase serum levels of dextromethorphan.
Corydalis yanhusuo contains berberine. Preliminary clinical research shows that berberine can inhibit cytochrome P450 2D6 (CYP2D6) activity and reduce the metabolism of dextromethorphan. Theoretically, Corydalis yanhusuo may also inhibit the metabolism of dextromethorphan.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's a little wrinkle worth knowing about. The interaction on file actually points to dextromethorphan, the cough suppressant that's often paired with guaifenesin in combo products like Robitussin DM. Corydalis may contain berberine, which can theoretically slow down a liver enzyme called CYP2D6 that clears dextromethorphan, letting it build up a bit.

Now, guaifenesin itself isn't really affected this way. It mostly just thins mucus and is cleared without leaning on CYP2D6, so on its own it shouldn't be a concern here. The catch is if your product is a 2-in-1 with dextromethorphan, more of that ingredient could mean extra drowsiness or dizziness. This is theoretical, not proven in people, so don't panic, but please check your label and ask your pharmacist if it contains DM.
