Here's the basic idea with Lonafarnib (Zokinvy) and this herb. Corydalis contains berberine, which lab studies suggest can slow down an enzyme called CYP2C9. Since some of your body's enzymes help clear lonafarnib, blocking them could let the drug build up a bit higher than expected. Lonafarnib is an active medicine (not a prodrug), so more of it in your system usually means a greater chance of its typical side effects, like nausea, vomiting, diarrhea, poor appetite, and fatigue.

I want to be honest though, this concern is theoretical, based on pharmacology rather than real human studies pairing these two. Still, because lonafarnib is a specialty medicine for a serious condition, please check with your doctor or pharmacist before adding corydalis.

Here's the basic idea with Zokinvy (lonafarnib). Your liver uses an enzyme called CYP3A4 to clear this drug out of your body. Corydalis contains berberine, which lab and early human studies suggest can partly block that same enzyme. If it slows the cleanup, more lonafarnib could linger in your blood, and that may mean stronger side effects like nausea, vomiting, diarrhea, loss of appetite, or fatigue.

I want to be straight with you: this is a theoretical concern based on how these compounds behave, not proven cases in people. Still, lonafarnib is a sensitive drug with careful dosing, so I wouldn't combine them on your own. Please check with your pharmacist or doctor first.

About Lonafarnib

What Is Lonafarnib Used For?

Brand name: Zokinvy

Zokinvy (lonafarnib) is approved for patients 12 months of age and older who have a body surface area of 0.39 m² or above. It is used to reduce the risk of mortality in Hutchinson-Gilford Progeria Syndrome (HGPS) — a rare, severe premature-aging disorder.

Zokinvy is also used to treat processing-deficient Progeroid Laminopathies — a related group of rare genetic conditions — specifically those caused by a heterozygous LMNA mutation with progerin-like protein buildup, or by homozygous or compound heterozygous ZMPSTE24 mutations. It is not indicated for other Progeroid Syndromes or processing-proficient Progeroid Laminopathies.
