Why this applies to Lansoprazole, Amoxicillin, Clarithromycin

Lansoprazole, Amoxicillin, Clarithromycin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cubebs interacts with.

Several compounds isolated from cubebs can inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this effect has not been reported in humans. Theoretically, cubebs might increase the effects and side effects of drugs metabolized by CYP3A4. These include lovastatin (Mevacor), clarithromycin (Biaxin), cyclosporine (Neoral, Sandimmune), diltiazem (Cardizem), estrogens, indinavir (Crixivan), triazolam (Halcion), and numerous others.

This Prevpac combo treats H. pylori stomach infections, and the piece worth watching here is the clarithromycin. Your body clears clarithromycin partly through an enzyme called CYP3A4. In the lab, certain compounds in cubebs appear to slow that enzyme down. In theory, if that enzyme works less well, more clarithromycin could linger in your system, which might mean a bit more nausea, an upset stomach, or a strange metallic taste.

But here's the honest part: this is only seen in test-tube studies, not in real people, so the risk is mostly theoretical. A sprinkle of cubebs as a kitchen spice is very unlikely to matter. Concentrated cubeb oils or extracts are the bigger question mark, so check with your pharmacist or doctor before using those.

PROTON PUMP INHIBITORS (PPIs)

Lansoprazole, Amoxicillin, Clarithromycin also falls under PROTON PUMP INHIBITORS (PPIs).

Theoretically, due to reports that cubebs increases stomach acid, cubebs might decrease the effectiveness of PPIs. PPIs include omeprazole (Prilosec), lansoprazole (Prevacid), rabeprazole (Aciphex), pantoprazole (Protonix), and esomeprazole (Nexium).

This combo pack (Prevpac) pairs the acid blocker lansoprazole with two antibiotics, amoxicillin and clarithromycin, to clear an H. pylori stomach infection. The lansoprazole piece is the part that matters here. Cubebs have a traditional reputation for stirring up stomach acid, and in theory that could work against lansoprazole, whose whole job is to lower acid. If acid creeps back up, you might notice more heartburn or a less effective treatment.

Honestly, this is a very weak, theory-based concern built on anecdotes, not solid studies. A sprinkle of cubeb as a spice is unlikely to cause trouble. Concentrated cubeb extracts or oils are the bigger question mark, so check with your pharmacist or doctor before using those while on Prevpac.
