First, please know that Datura wrightii is genuinely dangerous and isn't something I'd ever recommend swallowing in any amount. Setting that aside, here's the concern with Duranest (etidocaine combined with epinephrine), which is a local anesthetic dentists and surgeons use to numb an area for a procedure.

The worry on file is that datura can slow your breathing, and certain anesthetic situations can do the same, so the two together might add up and depress breathing in a way that could be serious. Honestly, this is based on weak, anecdotal evidence rather than solid studies. Etidocaine isn't a prodrug, so this isn't about one speeding up or slowing down the other in the liver. The bigger picture is simply that datura is toxic on its own. Always tell your dentist or doctor about everything you've taken before any procedure.
