Theoretically, evodia might have an additive effect with drugs that prolong the QT interval, potentially increasing the risk of ventricular arrhythmias.
Evodia has demonstrated dose-dependent activity as a proarrhythmic agent in animal and in vitro studies. Evodia infusion in animals extends the action duration potential and induces prolongation of the QT interval and Torsade de pointes.

Here's the concern with pairing Evodia and Isturisa (osilodrostat). Osilodrostat is already known to affect your heart's electrical rhythm in a way doctors call QT prolongation, especially at higher doses. Evodia, in lab and animal studies, has shown the same kind of effect. Stacking two things that both lengthen the QT interval could, in theory, raise the chance of a dangerous irregular heartbeat.

I want to be honest about how soft this evidence is. The Evodia data comes only from test tubes and animals, not people, so the real risk in humans isn't well known. Still, since osilodrostat itself carries a real QT warning, I'd play it safe. Please check with your doctor or pharmacist before combining them, and watch for fainting, dizziness, or a fluttering heartbeat.

Osilodrostat Phosphate also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, evodia might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that evodia extract inhibits hepatic CYP3A4. This effect has not been reported in humans.

Here's the gist for Isturisa (osilodrostat), the medicine you take to lower cortisol in Cushing's disease. Osilodrostat is partly cleared by a liver enzyme called CYP3A4. In test tube studies, evodia appears to slow that same enzyme down. If that happened in your body too, osilodrostat could clear a bit slower and build up, which might mean a stronger drug effect and more side effects like low cortisol symptoms (tiredness, nausea, dizziness, low blood pressure), low potassium, or QT changes on an EKG.

The honest part: this is only theoretical, based on lab research, not real people. It may never show up in practice. Still, since osilodrostat is dose-sensitive, please check with your doctor or pharmacist before pairing them.
