Puravive contains Panax ginseng. Panax ginseng is thought to have an additive effect with drugs that prolong the QT interval, potentially increasing the risk of ventricular arrhythmias. However, research findings on this matter are conflicting. Here's the thing to know about pairing Puravive with Isturisa (osilodrostat). Puravive contains Panax ginseng, and osilodrostat is already known to stretch out the heart's electrical reset, what we call the QT interval. Stacking the two could, in theory, add up and slightly raise the risk of an abnormal heart rhythm. Warning signs would be things like fainting, palpitations, dizziness, or a fluttering heartbeat.

I want to be honest though: the evidence here is on the weaker side. Some short-term ginseng studies showed a small QT change, while longer use showed none, so the findings conflict. This isn't about osilodrostat being broken down differently, it's just an overlapping effect. Please run it by your doctor or pharmacist before adding Puravive, since osilodrostat patients often have their heart tracing monitored anyway.

Osilodrostat Phosphate also falls under Cytochrome P450 3A4 (Cyp3A4) Substrates.

Puravive contains Panax ginseng. Panax ginseng has the potential to affect the breakdown of drugs that are metabolized by an enzyme called CYP3A4. One example of such a drug is imatinib. There was a case where imatinib was thought to have caused liver damage, and it was believed that this might have been due to Panax ginseng inhibiting CYP3A4, which is involved in imatinib's metabolism (https://pubmed.ncbi.nlm.nih.gov/20332334/). On the other hand, Panax ginseng has been found to increase the breakdown of another drug called midazolam, which is also metabolized by CYP3A4 (https://pubmed.ncbi.nlm.nih.gov/21646440/), suggesting Panax ginseng induces CYP3A4 in some cases (i.e., makes more potent). Given these contradictory findings, it is important to use Panax ginseng cautiously if you are taking medications that are metabolized by CYP3A4, as the interaction between Panax ginseng and these drugs is not fully understood.

Here's the situation with Isturisa (osilodrostat), which treats Cushing's disease by lowering cortisol. One of the ingredients in Puravive is Panax ginseng, and ginseng can nudge an enzyme called CYP3A4 that helps process osilodrostat. The tricky part is that studies disagree: sometimes ginseng seems to slow this enzyme down, and sometimes it speeds it up. So we honestly can't predict which way your osilodrostat levels would move.

If levels crept up, you might see more dizziness, fatigue, low cortisol symptoms, or heart rhythm changes. If they dropped, your cortisol control could slip. The good news is this interaction is rated minor with weak evidence, so it's more a caution than an alarm. Please don't change anything on your own. Just check with your doctor or pharmacist before adding Puravive.
