Osilodrostat Phosphate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Chanca Piedra interacts with.

Theoretically, use of chanca piedra might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that chanca piedra extract inhibits CYP3A4. Theoretically, chanca piedra might increase the levels of CYP3A4 substrates. This interaction has not been reported in humans.

Here's the gist for Isturisa (osilodrostat), the medicine that lowers cortisol in Cushing's disease. Your body partly clears osilodrostat using a liver enzyme called CYP3A4. In lab (test tube) studies, chanca piedra appears to slow that enzyme down. If that same effect happened in people, osilodrostat could build up a bit more than expected and push its effects harder.

For osilodrostat, that might mean cortisol dropping too low (fatigue, nausea, dizziness, low blood pressure) or a higher chance of low potassium or QT changes on an EKG. I want to be honest, though: this is only theoretical from lab work and hasn't been seen in real patients. Still, since osilodrostat is dose-sensitive, please run it by your doctor or pharmacist before combining them.
