Why this applies to Abemaciclib

Abemaciclib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldenseal interacts with.

Goldenseal might increase serum levels of drugs metabolized by CYP3A4.
Most clinical and in vitro research shows that goldenseal inhibits CYP3A4 enzyme activity and increases serum levels of CYP3A4 substrates, such as midazolam. However, in one small clinical study, goldenseal did not affect the levels of indinavir, a CYP3A4 substrate, in healthy volunteers. This is likely due to the fact that indinavir has a high oral bioavailability, making it an inadequate probe for CYP3A4 interactions and/or that it is primarily metabolized by hepatic CYP3A, while goldenseal has more potential to inhibit intestinal CYP3A enzyme activity. Both goldenseal extract and its isolated constituents berberine and hydrastine inhibit CYP3A, with hydrastine possibly having more inhibitory potential than berberine.

Here's the short version: your body relies on an enzyme called CYP3A4 to break down and clear abemaciclib (Verzenio). Goldenseal appears to slow that enzyme down. When that pathway is blocked, abemaciclib isn't cleared as quickly, so its levels in your blood can climb higher than intended.

For a cancer medicine like this, more drug usually means more side effects rather than better treatment. With abemaciclib that could mean worse diarrhea, nausea, fatigue, or bigger drops in your blood counts (which can raise infection risk). The human evidence here is moderate, not ironclad, but I wouldn't take chances with a chemotherapy-type drug. Please don't start or stop anything on your own. Just loop in your oncologist or pharmacist before adding goldenseal.

About Abemaciclib

What Is Abemaciclib Used For?

Brand name: Verzenio

Verzenio (abemaciclib) is used to treat HR-positive, HER2-negative breast cancer in adults. For early breast cancer at high risk of recurrence, Verzenio is taken alongside endocrine therapy (tamoxifen or an aromatase inhibitor) after surgery to help prevent the cancer from coming back.

For advanced or metastatic breast cancer (cancer that has spread), Verzenio can be used in combination with an aromatase inhibitor as a first hormonal treatment, in combination with fulvestrant after the cancer has progressed on endocrine therapy, or alone after prior endocrine therapy and chemotherapy have stopped working.
