Here's the quick picture: goldenseal can slow down a liver enzyme called CYP2D6, and your body uses that same enzyme to help convert dolasetron into its active form. So when goldenseal blocks CYP2D6, you may actually get a bit less of the active anti-nausea compound, not more. In plain terms, your dolasetron might not work quite as well at keeping chemo-related nausea and vomiting under control.

This is based on lower-quality human evidence, so it's a real possibility rather than a sure thing. Concentrated goldenseal supplements are more likely to matter than tiny amounts. Since controlling nausea during chemo is important, please check with your oncology pharmacist or doctor before mixing the two.

Goldenseal might increase serum levels of drugs metabolized by CYP3A4.
Most clinical and in vitro research shows that goldenseal inhibits CYP3A4 enzyme activity and increases serum levels of CYP3A4 substrates, such as midazolam. However, in one small clinical study, goldenseal did not affect the levels of indinavir, a CYP3A4 substrate, in healthy volunteers. This is likely due to the fact that indinavir has a high oral bioavailability, making it an inadequate probe for CYP3A4 interactions and/or that it is primarily metabolized by hepatic CYP3A, while goldenseal has more potential to inhibit intestinal CYP3A enzyme activity. Both goldenseal extract and its isolated constituents berberine and hydrastine inhibit CYP3A, with hydrastine possibly having more inhibitory potential than berberine.

Here's the gist for Anzemet (dolasetron), the anti-nausea medicine you take around chemotherapy. Goldenseal tends to slow down a liver and gut enzyme called CYP3A4, which is one of the pathways your body uses to process dolasetron. When that pathway is blocked, the drug can clear a little more slowly and build up higher than expected.

In practice that could mean a bit more of dolasetron's usual side effects, like headache, dizziness, or tiredness. The bigger thing pharmacists watch for with 5-HT3 medicines is heart rhythm changes (a longer QT interval), so I'd be cautious stacking anything that nudges levels upward. This is a moderate, possible interaction based on limited human data, not a sure thing. Please check with your oncology team or pharmacist before using goldenseal.
