Mannitol falls under ANTIHYPERTENSIVE DRUGS — the group Goldenseal interacts with.

Theoretically, goldenseal might increase the risk of hypotension when taken with antihypertensive drugs.
Goldenseal contains berberine. Animal research shows that berberine can have hypotensive effects. Also, an analysis of clinical research shows that taking berberine in combination with amlodipine can lower systolic and diastolic blood pressure when compared with amlodipine alone. However, this effect has not been reported with goldenseal.

Here's the gist: goldenseal contains berberine, and in animal studies (and some human work with blood pressure pills) berberine can nudge blood pressure downward. Mannitol works differently from a typical blood pressure medicine. It's an osmotic agent that pulls fluid out of tissues and into the urine, which can lower pressure in the brain or eyes, but it also shifts a lot of fluid around and can drop your blood pressure too.

The worry is mostly theoretical, based on how these two might add up rather than real reports. Still, stacking goldenseal on top of mannitol could in theory mean more lightheadedness, dizziness, or low blood pressure. Mannitol is usually given in a hospital setting, so let your care team know about any goldenseal you take.
