Omeprazole, Sodium Bicarbonate falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Goldenseal interacts with.

Theoretically, goldenseal might increase serum levels of drugs metabolized by CYP2C9.
In vitro research shows that goldenseal root extract can modestly inhibit CYP2C9. This effect may be due to its alkaloid constituents, hydrastine and berberine. However, this effect has not been reported in humans.

Here's the gist with goldenseal and your omeprazole/sodium bicarbonate combo (Konvomep). Goldenseal contains alkaloids like berberine and hydrastine that, in lab dish studies, can mildly slow down a liver enzyme called CYP2C9. Omeprazole is partly handled by that enzyme, so in theory the herb could let a bit more omeprazole linger in your system. That might nudge up common side effects like headache, nausea, gas, or diarrhea.

The important part: this has only been seen in test-tube research, never confirmed in real people. So I wouldn't lose sleep over it. Still, since goldenseal can affect several medications, it's worth a quick chat with your pharmacist or doctor before adding it. Don't change your omeprazole on your own.

Omeprazole, Sodium Bicarbonate also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Goldenseal might increase serum levels of drugs metabolized by CYP3A4.
Most clinical and in vitro research shows that goldenseal inhibits CYP3A4 enzyme activity and increases serum levels of CYP3A4 substrates, such as midazolam. However, in one small clinical study, goldenseal did not affect the levels of indinavir, a CYP3A4 substrate, in healthy volunteers. This is likely due to the fact that indinavir has a high oral bioavailability, making it an inadequate probe for CYP3A4 interactions and/or that it is primarily metabolized by hepatic CYP3A, while goldenseal has more potential to inhibit intestinal CYP3A enzyme activity. Both goldenseal extract and its isolated constituents berberine and hydrastine inhibit CYP3A, with hydrastine possibly having more inhibitory potential than berberine.

Here's the gist with Konvomep (omeprazole plus sodium bicarbonate): goldenseal can slow down CYP3A4, one of the liver enzymes that helps clear omeprazole from your body. When that enzyme is partly blocked, a bit more omeprazole can linger in your bloodstream. In practice, this could mean slightly stronger acid-blocking effects or a touch more of the usual nuisances like headache, nausea, gas, or loose stools.

The good news is that omeprazole is generally well tolerated, so a modest bump usually isn't dangerous. Still, the evidence here is only moderate, and goldenseal also affects gut enzymes more than liver ones. If you take goldenseal regularly, just mention it to your pharmacist or doctor before combining the two.
