Hydrochlorothiazide, Losartan Potassium falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldthread interacts with.

Here's the honest picture with Hyzaar (losartan plus hydrochlorothiazide) and goldthread. The concern on file is that berberine, goldthread's main active compound, may block a liver enzyme called CYP3A4 in lab settings. Losartan does get partly processed by that enzyme, but it's actually a prodrug, meaning your body converts it into its main active form. If that conversion is slowed, you could in theory get a little less blood pressure lowering from the losartan side. The water pill (hydrochlorothiazide) isn't really affected by this enzyme.
The reassuring part: this is only a theoretical, lab-based idea and hasn't been reported in real patients. Still, if you want to try goldthread, run it by your pharmacist or doctor first and keep an eye on your home blood pressure readings.
