Acetaminophen, Butalbital, Caffeine, Codeine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Grapefruit interacts with.

Grapefruit juice can increase levels of drugs metabolized by CYP3A4.
Clinical research shows that grapefruit juice can inhibit CYP3A4 metabolism of drugs, causing increased drug levels and potentially increasing the risk of adverse effects. When taken orally, effects of grapefruit juice on CYP3A4 levels appear to last at least 48 hours. Grapefruit's ability to inhibit CYP3A4 has even been harnessed to intentionally increase levels of venetoclax, which is metabolized by CYP3A4, in an elderly patient with acute myeloid leukemia who could not afford full dose venetoclax. The lower dose of venetoclax in combination with grapefruit juice resulted in serum levels of venetoclax in the therapeutic reference range of full dose venetoclax and positive treatment outcomes for the patient.
Professional consensus recommends the consideration of patient age, existing medical conditions, additional medications, and the potential for additive adverse effects when evaluating the risks of concomitant use of grapefruit juice with any medication metabolized by CYP3A4. While all patients are at risk for interactions with grapefruit juice consumption, patients older than 70 years of age and those taking multiple medications are at the greatest risk for a serious or fatal interaction with grapefruit juice.

This combo pill is a bit complicated because it has four ingredients. The one I'd watch most with grapefruit is the butalbital, a barbiturate that your liver partly clears using the CYP3A4 enzyme. Grapefruit blocks that enzyme, which can let butalbital build up higher than expected. That could mean extra drowsiness, dizziness, slowed breathing, or feeling very groggy, especially if you're older or take other sedating medicines.

The codeine part is a little different. Codeine is a prodrug that has to be activated by a separate enzyme (CYP2D6), so grapefruit doesn't really boost its pain relief. Caffeine and acetaminophen aren't strongly affected here either. Bottom line: grapefruit's biggest concern is added sedation from the butalbital. Please check with your pharmacist or doctor before mixing them regularly.

This combination pill has four parts, and the one grapefruit may touch is the caffeine. Caffeine is cleared by a liver enzyme called CYP1A2, and lab studies hint that grapefruit might slow that enzyme down a little. If that happened in real life, caffeine could linger longer and you might feel jittery, have a racing heart, or trouble sleeping.

Here is the reassuring part: this effect has only been seen in test tubes, never confirmed in actual people, so I would not lose sleep over it. Grapefruit's bigger concern with this medicine is the codeine and butalbital side (drowsiness and sedation), so it is still smart to run your full medication list past your pharmacist or doctor.

The piece of this combination pill that grapefruit really touches is the caffeine. Grapefruit can slow down how fast your body clears caffeine, so it tends to hang around a bit longer and feel a little stronger than usual. In real life that might mean more jitteriness, a racing or pounding heart, trouble sleeping, or feeling on edge.

Keep in mind this concern is mostly based on how the drugs behave on paper and in small studies, not on dramatic real-world reports, so for most people it is a mild effect. An occasional grapefruit is unlikely to cause trouble, but if you drink grapefruit juice often or feel wired, it is worth a quick chat with your pharmacist or doctor.
