Trovafloxacin falls under ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP) — the group Grapefruit interacts with.

Grapefruit juice can decrease levels of drugs that are substrates of OATP.
In vitro and clinical research show that consuming grapefruit juice inhibits OATP, which reduces the bioavailability of oral drugs that are substrates of OATP. Various clinical studies have shown reduced absorption of OATP substrates when taken with grapefruit, including fexofenadine, acebutolol, aliskiren, celiprolol, levothyroxine, nadolol, and pitavastatin. Grapefruit juice is thought to affect OATP for only a short time. Therefore, separating drug administration and consumption of grapefruit by at least 4 hours is likely to prevent this interaction.

Here's the gist: grapefruit can briefly block a gut transporter called OATP that helps certain medicines get absorbed. Trovafloxacin is an antibiotic, and the worry is that grapefruit could lower how much of the drug your body actually takes in. With less antibiotic on board, you may not knock out the infection the way the drug is meant to, which can mean a poor response or even treatment failure.

The good news is this OATP effect is short-lived, so spacing things out by at least 4 hours usually avoids the problem. Keep in mind the evidence here is moderate and based mostly on other OATP drugs, not trovafloxacin itself. When you're fighting an infection, full absorption matters, so please run your grapefruit habit by your pharmacist or doctor first.
