Larotrectinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Green Tea interacts with.

Green tea is unlikely to produce clinically significant changes in the levels and clinical effects of CYP3A4 substrates.
In vitro and in vivo research suggests that green tea can inhibit intestinal CYP3A and induce hepatic CYP3A4 enzymes. However, this effect is unlikely to be clinically significant, as green tea does not appear to affect CYP3A4 activity in humans.

Here's the short version: your body clears larotrectinib (Vitrakvi) using a liver enzyme called CYP3A4, and green tea can tug on that same enzyme. In a test tube green tea seems to slow CYP3A4 in the gut but speed it up in the liver. On paper, slowing it could let more drug build up (more side effects like dizziness, fatigue, nausea, or numbness/tingling), while speeding it up could lower drug levels. The good news is that in actual human studies, green tea hasn't been shown to meaningfully change CYP3A4 activity.

So a cup or two of regular green tea is very unlikely to cause trouble here. The bigger caution is with concentrated green tea extract supplements, which can rarely stress the liver. Since this is a cancer medicine with a narrow margin, please run any supplement past your oncology pharmacist or doctor first.

About Larotrectinib

What Is Larotrectinib Used For?

Brand name: Vitrakvi

Vitrakvi is used to treat adult and pediatric patients with solid tumors that have a neurotrophic receptor tyrosine kinase (NTRK) gene fusion — a specific genetic alteration — without a known acquired resistance mutation. It is used when the tumor has spread (is metastatic) or when surgery would likely cause severe harm, and when there are no satisfactory alternative treatments or the cancer has continued to grow after prior treatment.

Patients are selected for Vitrakvi based on an FDA-approved test that confirms the NTRK gene fusion in tumor tissue. This approval was granted under the FDA's accelerated approval pathway, based on overall response rate and duration of response; continued approval may depend on results from confirmatory trials.
