Grepafloxacin falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Hibiscus Sabdariffa interacts with.

Theoretically, Hibiscus sabdariffa extract might reduce the metabolism of CYP1A2 substrates.
In vitro research shows that Hibiscus sabdariffa calyx extract inhibits CYP1A2. This interaction has not been reported in humans.

Here's the gist with hibiscus and grepafloxacin. Grepafloxacin is an antibiotic that your liver partly clears using an enzyme called CYP1A2. In a lab dish, hibiscus extract slowed that same enzyme down. If that happened in your body, grepafloxacin could be cleared a little more slowly and build up slightly, which in theory might nudge up side effects like nausea, dizziness, or effects on your heart's rhythm.

I want to keep this in perspective, though. This is a theoretical concern from test-tube work only, and it has never actually been reported in people. A cup or two of hibiscus tea is unlikely to matter much, while concentrated supplements are the bigger question mark. If you're taking this medicine, just check with your pharmacist or doctor before adding a hibiscus supplement.
