Theoretically, taking Hibiscus sabdariffa with antihypertensive drugs might increase the risk of hypotension.
Most clinical evidence suggests that taking Hibiscus sabdariffa reduces systolic and diastolic blood pressure. In animal research, Hibiscus sabdariffa increased the hypotensive effects of single doses of losartan and amlodipine.

Here's the thing to know if you're taking Opsumit (macitentan) for pulmonary arterial hypertension. Macitentan already relaxes and opens up blood vessels, and that can gently lower your blood pressure on its own. Hibiscus, especially in concentrated supplement form, has been shown to nudge blood pressure down too. So when you stack the two, the worry is simply that their effects add up and your blood pressure could drop a little too far.

If that happened, you might notice dizziness, lightheadedness, feeling faint when you stand up, or unusual tiredness. To be honest, this concern is mostly theoretical and based on how these things work, not on strong studies with this exact drug. A cup of tart hibiscus tea now and then is unlikely to cause trouble, but check with your pharmacist or doctor before using daily hibiscus supplements.

Macitentan also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, Hibiscus sabdariffa extract might reduce the metabolism of CYP3A4 substrates.
In vitro research shows that Hibiscus sabdariffa calyx extract inhibits CYP3A4. This interaction has not been reported in humans.

Here's the situation with hibiscus and Opsumit (macitentan). Your liver uses an enzyme called CYP3A4 to help clear macitentan from your body. In lab (test tube) studies, concentrated hibiscus extract appeared to slow that enzyme down a bit. If that happened in real life, a little more macitentan could linger in your system, which in theory might nudge up side effects like headache, stuffy nose, swelling in the legs, low blood pressure, or feeling more tired.

I want to keep this in perspective. This is theoretical and based only on lab work, not anything seen in actual patients. A normal cup of tart hibiscus tea is very unlikely to cause trouble. Concentrated hibiscus supplements are where I'd be more careful, so just run it by your doctor or pharmacist first.

What Is Macitentan Used For?

Brand name: Opsumit

Macitentan tablets — sold under the brand name Opsumit — are used to treat pulmonary arterial hypertension (PAH, WHO Group I), a serious condition where the blood pressure in the arteries supplying the lungs is abnormally high. The goal is to slow down how quickly the disease gets worse and to lower the chances of being hospitalized because of it.

In clinical studies, it was shown to be effective in adults with mostly moderate-to-severe PAH symptoms. Patients in those studies had PAH caused by unknown or inherited reasons, connective tissue disorders, or congenital heart disease with repaired heart shunts.
