Theoretically, taking Hibiscus sabdariffa with antihypertensive drugs might increase the risk of hypotension.
Most clinical evidence suggests that taking Hibiscus sabdariffa reduces systolic and diastolic blood pressure. In animal research, Hibiscus sabdariffa increased the hypotensive effects of single doses of losartan and amlodipine.

Here's the gist: riociguat already works by relaxing your blood vessels to ease the pressure in your lungs, and a known side effect is that it can drop your overall blood pressure too. Hibiscus tea has a mild blood-pressure-lowering effect of its own, so the worry is that stacking the two could push your numbers lower than intended. That might show up as dizziness, lightheadedness, feeling faint when you stand up, or headache.

I want to be honest about the evidence here: this concern is theoretical, based on how both work, with some animal studies but no solid human proof for this exact combo. An occasional cup of hibiscus tea is unlikely to cause trouble, but concentrated hibiscus supplements are a bigger question mark. Please check with your pharmacist or doctor before adding it.

Riociguat also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, Hibiscus sabdariffa extract might reduce the metabolism of CYP3A4 substrates.
In vitro research shows that Hibiscus sabdariffa calyx extract inhibits CYP3A4. This interaction has not been reported in humans.

Here's the gist with Adempas (riociguat) and hibiscus. Your liver uses an enzyme called CYP3A4 to help clear riociguat from your body. In lab (test tube) studies, hibiscus extract appears to slow that enzyme down a bit. If that happened in real life, your body would clear riociguat a little slower, so more could build up and its effects could get stronger.

Since riociguat already lowers blood pressure, the main thing to watch for would be too much blood pressure lowering: dizziness, lightheadedness, headache, or feeling faint when you stand up. The good news is this is only a theoretical concern that has never been reported in people, and an occasional cup of hibiscus tea is unlikely to matter much. Still, before using concentrated hibiscus supplements, check with your pharmacist or doctor.

About Riociguat

What Is Riociguat Used For?

Brand name: Adempas

Riociguat tablets — sold as Adempas and under the generic name Riociguat — are used to treat adults with two types of pulmonary hypertension (high blood pressure in the lungs).

The first is chronic thromboembolic pulmonary hypertension (CTEPH), a condition caused by blood clots in the lung arteries, either after surgery or when surgery is not an option. The second is pulmonary arterial hypertension (PAH), where the arteries carrying blood to the lungs are narrowed or blocked. In both conditions, riociguat helps improve exercise ability and functional status, and in PAH it also helps slow the disease from getting worse.
