Theoretically, higenamine might increase the levels and clinical effects of drugs metabolized by CYP2D6.
In vitro research shows that higenamine inhibits CYP2D6 enzymes. However, this effect has not been reported in humans.

Here's the basic idea with methadone and higenamine. Lab studies suggest higenamine can block an enzyme called CYP2D6, which is one of the pathways your body uses to clear methadone. If that pathway slows down, methadone could build up a little more than expected. Since methadone is an active drug (not a prodrug that needs activating), more of it in your system could mean stronger effects like deeper drowsiness, slowed breathing, constipation, or low blood pressure.

I want to be honest about the evidence though: this is only seen in test-tube research, not in real people, so it's mostly theoretical. Still, methadone is a serious medication with a narrow safety margin, and higenamine itself can stir up your heart and blood pressure. Please run this combination by your pharmacist or doctor before mixing them.

Here's the gist for methadone: your liver uses an enzyme called CYP3A4 to help break it down and clear it from your body. In lab dish studies, higenamine slowed that same enzyme down a little (about 21%). If that happened in real life, methadone could build up more than expected.

Why does that matter? Methadone is a powerful opioid with a narrow safety window. Higher levels can mean more sedation, slowed breathing, and effects on your heart rhythm. The honest truth, though, is that this is only seen in a test tube, not in actual people, so we can't say it really happens. About Methadone

What Is Methadone Used For?

Brand name: Dolophine

Methadone treats severe and persistent pain that requires daily, around-the-clock, long-term opioid treatment and cannot be managed adequately with other options. It is available in several forms for this use, including oral solution, oral tablets, oral concentrate, and injectable forms (intravenous, intramuscular, and subcutaneous). Injectable methadone is not approved for outpatient pain treatment — it is reserved for hospitalized patients unable to take oral medication.

Methadone is also used for the detoxification and maintenance treatment of opioid addiction (dependence on heroin or other morphine-like drugs). Oral forms — including tablets, dispersible tablets (such as Methadose Dispersible and DISKETS), and oral concentrates (such as Methadone Hydrochloride Intensol™, Methadose, and others) — are approved for this use. By law, methadone for opioid addiction treatment must be dispensed only by certified opioid treatment programs. Injectable methadone is not approved for outpatient addiction treatment.
