Codeine Phosphate, Pseudoephedrine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Higenamine interacts with.

Theoretically, higenamine might increase the levels and clinical effects of drugs metabolized by CYP2D6.
In vitro research shows that higenamine inhibits CYP2D6 enzymes. However, this effect has not been reported in humans.

Here's the wrinkle with this combo: codeine is a prodrug. Your body has to use an enzyme called CYP2D6 to turn codeine into morphine, which is the part that actually relieves pain and cough. Higenamine has been shown in lab dishes to block CYP2D6. If that blocking happened in real people, you'd convert less codeine into morphine, so you could get less pain or cough relief than expected.

The good news is this is only theoretical right now, seen in test tubes and not in humans, so I wouldn't lose sleep over it. Higenamine itself is also a heart stimulant, and pseudoephedrine raises heart rate and blood pressure too, so stacking them could mean jitters, a racing heart, or higher blood pressure. I'd run this past your pharmacist before combining them.

Theoretically, higenamine might increase the risk of cardiovascular toxicity when taken with stimulant drugs.
Higenamine has stimulant effects due to agonist activity at beta2-adrenoreceptors. In cardiac muscle, higenamine appears to have a positive inotropic effect and increase heart rate. However, it does not appear to increase blood pressure.

Here's the thing to focus on with this combo: the pseudoephedrine part of your medicine is itself a stimulant. It can already speed up your heart and bump your blood pressure. Higenamine works on similar adrenaline-type receptors, so stacking the two could pile on those stimulant effects. You might notice a racing or pounding heart, jitteriness, trouble sleeping, or feeling wired. People with heart issues or high blood pressure should be especially careful.

I'll be honest, this concern is theoretical, based on how these compounds work rather than solid human studies. Higenamine doses on labels are also often inaccurate. Before mixing them, please run it by your pharmacist or doctor.
