Theoretically, taking lithium supplements with NSAIDs might increase lithium levels and adverse effects.
NSAIDs can decrease the renal clearance of lithium and increase lithium levels. It is unclear if this interaction would be clinically significant with the smaller doses found in lithium supplements.

The piece to focus on here is the aspirin in this combination product. Aspirin is an NSAID, and NSAIDs can slow how fast your kidneys clear lithium out of your body. When lithium clears more slowly, its level in your blood can creep up, and lithium is a drug where even small changes matter. Higher lithium levels can bring on things like nausea, hand tremor, increased thirst, frequent urination, and feeling shaky or unsteady.

That said, this is a theoretical concern based on how the drugs work, and it's really not clear it matters with the low amounts in lithium supplements. The caffeine and hydrocodone aren't the issue. Still, please check with your pharmacist or doctor before combining these, especially if you take prescription lithium.

Xanthines such as aminophylline, caffeine, and theophylline (Theo-Dur, Theo-24, others) might increase the clearance of lithium. It is unclear if this interaction would be clinically significant with the smaller doses found in lithium supplements.

The piece that matters here is the caffeine in this combination product (which also contains aspirin and hydrocodone for pain). Caffeine is a methylxanthine, and in theory it can make your kidneys flush lithium out a little faster. If that happened, your lithium levels could dip and you might get less of the mood-stabilizing benefit you are counting on.

Honestly, this is a theoretical concern based on how these drugs behave, not strong human proof, and the small amount of caffeine in one of these tablets is unlikely to throw off carefully dosed lithium. Still, since lithium has a narrow safe range, it is worth a quick chat with your pharmacist or doctor before mixing them regularly.
