Promethazine falls under PHENOTHIAZINES — the group Lithium interacts with.

Theoretically, taking lithium supplements with phenothiazines might decrease the levels and clinical effects of phenothiazines.
Concomitant use of lithium with phenothiazines might reduce lithium concentrations. Lithium might also reduce phenothiazine concentrations, making the pharmacokinetic effect unpredictable. It is unclear if these interactions would occur with the smaller doses found in lithium supplements.

Here's the gist for combining lithium with promethazine (Phenergan). On paper, the two may lower each other's levels. For promethazine, that could mean it works a little less well, so the antihistamine, anti-nausea, or sedative effect you're counting on might feel weaker than expected. Lithium levels could dip too, which matters because lithium has a narrow safety window where small shifts count.

I want to be honest though: this is a theoretical concern based on how the drugs behave, not strong real-world data. Promethazine isn't a prodrug, so it doesn't need activating. Most of the worry comes from prescription-strength lithium, and it's unclear whether low-dose lithium supplements do anything here at all. Still, before pairing them, please run it by your pharmacist or doctor.

About Promethazine

What Is Promethazine Used For?

Brand name: Phenergan

Promethazine is used for a wide range of conditions. Tablets, oral solution, and rectal suppositories (brands including Phenergan, Promethegan, and Phenadoz) are approved for seasonal and perennial allergic rhinitis, vasomotor rhinitis, allergic conjunctivitis, hives (urticaria), angioedema, dermographism, and allergic reactions to blood or plasma. They are also used for motion sickness (both prevention and active treatment), nausea and vomiting related to anesthesia and surgery, sedation, and as a helper medication alongside pain relievers after surgery.

Promethazine hydrochloride injection (brands including Phenergan and Promethazine HCl injection) is used for many of the same purposes but is reserved for situations where oral treatment is not possible — including allergic emergencies as an adjunct to epinephrine, sedation during labor, and as an aid during certain surgical procedures such as bronchoscopy and eye surgery.
