Ramipril falls under ACE INHIBITORS (ACEIs) — the group Lithium interacts with.

Theoretically, taking lithium supplements with ACEIs might increase levels and adverse effects of lithium.
Concomitant administration of ACEIs with lithium may increase lithium concentrations. It is unclear if this interaction would be clinically significant with the smaller doses found in lithium supplements.

Here's the gist for ramipril (Altace) and lithium: ACE inhibitors like ramipril can make your kidneys hold onto lithium, which can nudge lithium levels in your blood upward. When lithium climbs, you may notice things like hand tremors, nausea, increased thirst, more frequent urination, or feeling foggy and unsteady. Lithium has a narrow safety window, so even modest changes deserve attention.

That said, this is a theoretical concern based on how these drugs behave, and it's most relevant with prescription-strength lithium for bipolar disorder. The tiny amounts in over-the-counter lithium orotate supplements may not matter much, but we honestly don't know. Ramipril itself isn't a prodrug issue here, the concern is purely lithium buildup. Please loop in your pharmacist or doctor before combining them, especially if you take prescription lithium.

What Is Ramipril Used For?

Brand name: Altace

Ramipril capsules are used to treat high blood pressure (hypertension), either alone or combined with a thiazide diuretic (water pill). Lowering blood pressure reduces the risk of heart attack and stroke.

In adults 55 and older at high cardiovascular risk, ramipril capsules are used to reduce the risk of heart attack, stroke, or death from cardiovascular causes. Ramipril capsules are also used in stable patients who develop signs of heart failure within the first days after a heart attack, to lower the risk of death and hospitalization related to heart failure.
