Why Are There 2 Interactions?

ANTIHYPERTENSIVE DRUGS

Why this applies to Hydrochlorothiazide, Losartan Potassium

Hydrochlorothiazide, Losartan Potassium falls under ANTIHYPERTENSIVE DRUGS — the group Noni interacts with.

Theoretically, noni may increase the risk of hypotension when used in combination with antihypertensive drugs.
Preliminary clinical research suggests that drinking noni juice can reduce blood pressure in individuals with hypertension.

Hyzaar combines two blood pressure medicines, a water pill (hydrochlorothiazide) and an ARB (losartan). Here's the thing about noni juice: some early research hints that it can lower blood pressure on its own. So when you pair it with a medication already built to bring your pressure down, the two could add up and push your numbers a little too low.

If that happens, you might feel lightheaded, dizzy, tired, or woozy when you stand up quickly. To be clear, this is based on weak, anecdotal evidence, not strong proof, so it's more of a heads-up than a red flag. If you'd like to try noni, check with your pharmacist or doctor first and keep an eye on your blood pressure.

ANGIOTENSIN RECEPTOR BLOCKERS (ARBs)

2 of 2

Hydrochlorothiazide, Losartan Potassium also falls under ANGIOTENSIN RECEPTOR BLOCKERS (ARBs).

Theoretically, combining noni and ARBs might increase the risk of hyperkalemia.
Noni juice contains significant amounts of potassium, about 6 mEq/100 mL juice. This may increase the risk for hyperkalemia when used in conjunction with ARBs, which can also increase potassium levels.

Here's the thing about Hyzaar: it's a combo pill with losartan, an ARB that can nudge your potassium levels up, paired with hydrochlorothiazide, a water pill that tends to pull potassium down. The losartan side is what matters here. Noni juice is surprisingly rich in potassium (roughly 6 mEq per 100 mL), so drinking a lot of it on top of an ARB could push your potassium higher than you want.

To be honest, this is mostly a theoretical concern based on anecdotal reports, not strong studies, and the diuretic in your pill may partly offset it. Still, too much potassium can cause muscle weakness, tingling, or an irregular heartbeat. If you enjoy noni regularly, just mention it to your pharmacist or doctor so they can keep an eye on your potassium.

Noni is a tropical fruit used widely in folk medicine and sold mostly as a juice or supplement, but solid human evidence for its many claimed benefits is limited. While moderate juice intake is generally well tolerated, there are rare reports of liver problems, and it should be avoided in pregnancy and breastfeeding. Talk with your pharmacist or doctor before using it, especially if you have kidney or liver disease.
