These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Padang Cassia interacts with whole categories of medication. Acetaminophen, Aspirin, Caffeine falls into 2 of those categories, so it can interact with Padang Cassia in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
HEPATOTOXIC DRUGS
1 of 2Why this applies to Acetaminophen, Aspirin, Caffeine
Acetaminophen, Aspirin, Caffeine falls under HEPATOTOXIC DRUGS — the group Padang Cassia interacts with.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the concern with combining concentrated Padang cassia (a cassia-type cinnamon) and an Excedrin-style product. Padang cassia naturally contains coumarin, which in large amounts has been linked to liver stress. The acetaminophen part of Excedrin is also processed by the liver and is a well-known cause of liver injury when taken in high doses. Stacking two things that can tax the liver is the worry here.
To be clear, this is mostly a theoretical caution based on how these compounds behave, not strong human proof. Normal kitchen amounts of cinnamon are not a problem. But concentrated cinnamon capsules, especially alongside regular acetaminophen use or if you have liver issues or drink alcohol, are worth running by your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Acetaminophen, Aspirin, Caffeine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the practical side of this one. Lab tests suggest that concentrated Padang cassia (a cassia cinnamon) may slow down an enzyme called CYP3A4, which your body uses to help clear certain medicines. In theory, slowing that enzyme could let a drug build up a bit and make its effects, and side effects, stronger.
For this combo product (acetaminophen, aspirin, and caffeine), CYP3A4 isn't the main route your body uses to break these three down, so I wouldn't expect a big issue. The caffeine piece is the most relevant, and a slight buildup could mean more jitteriness, a racing heart, or trouble sleeping. Keep in mind this is only test-tube evidence, and sprinkling cinnamon on your oatmeal won't matter. If you take strong cinnamon capsules daily, just mention it to your pharmacist.

About Padang Cassia
What is Padang Cassia?
- Adding flavor to food
- Digestive upset
- Blood sugar support
- Antimicrobial purposes
- Antioxidant support
Padang Cassia is a type of cinnamon (Indonesian or Korintje cinnamon) grown mainly in Indonesia and used widely as a kitchen spice. As a supplement, evidence for health benefits like blood sugar control is limited and mixed, and like other cassia cinnamons it naturally contains coumarin, which can be harmful to the liver in large amounts. Use food amounts freely, but talk to a pharmacist or doctor before taking concentrated cinnamon supplements.
