Acetaminophen, Butalbital, Caffeine, Codeine falls under HEPATOTOXIC DRUGS — the group Padang Cassia interacts with.

There is some concern that ingesting large amounts of Padang cassia might cause hepatotoxicity in some people. Padang cassia contains coumarin. Coumarin has caused hepatotoxicity in animal models. In otherwise healthy humans, very high doses of coumarin, from 50-7000 mg daily, can result in hepatotoxicity that resolves when coumarin use is discontinued. Lower amounts might also cause liver problems in sensitive people, such as those with liver disease or those taking potentially hepatotoxic agents.

Here's the thing to keep an eye on with this combo. Your medication contains acetaminophen (Tylenol), which your liver has to process, and in large or frequent doses acetaminophen is itself a known cause of liver stress. Padang cassia is a cassia-type cinnamon that naturally contains coumarin, a compound that can also be hard on the liver when taken in concentrated amounts. Stacking two things that both lean on the liver is the concern here.

To be honest, this is more of a theoretical, careful-thinking concern than a proven reaction, and a sprinkle of cinnamon on your oatmeal is not going to matter. It's the concentrated cinnamon supplements taken daily that could add up, especially if you have any liver issues. Before starting a cinnamon supplement, just run it by your pharmacist or doctor.

In vitro research shows that a methanol extract of Padang cassia inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, taking Padang cassia with CYP3A4 substrates might increase the effects and side effects of these substrates.

This combination pill contains four ingredients, and a couple of them lean on the liver enzyme called CYP3A4. Test-tube studies suggest Padang cassia can block that enzyme a little. In theory, slowing it down could let some of these components, especially the butalbital and to a degree the codeine, hang around longer in your body. That might mean extra drowsiness, more dizziness, slowed breathing, or feeling unusually groggy.

Here's the honest part: this is only laboratory evidence, not human studies, so the real-world risk is probably small. Sprinkling cinnamon on food is not a concern. Concentrated cinnamon capsules are a different story, and they also carry coumarin, which is hard on the liver. Since this medicine already contains acetaminophen, please check with your pharmacist or doctor before adding cinnamon supplements.
