There is some concern that ingesting large amounts of Padang cassia might cause hepatotoxicity in some people. Padang cassia contains coumarin. Coumarin has caused hepatotoxicity in animal models. In otherwise healthy humans, very high doses of coumarin, from 50-7000 mg daily, can result in hepatotoxicity that resolves when coumarin use is discontinued. Lower amounts might also cause liver problems in sensitive people, such as those with liver disease or those taking potentially hepatotoxic agents.

The product you're asking about is a combination cold medicine, and the piece that matters here is the acetaminophen. Acetaminophen is one of the most common causes of liver strain when taken in high doses. Padang cassia is a cassia cinnamon, and it naturally carries a compound called coumarin that, in large amounts, has been linked to liver trouble. So the worry is simply two things that can both lean on the liver adding up.

This concern is theoretical and based mostly on pharmacology and animal data, so don't be alarmed. Sprinkling cinnamon on your food is not the issue. Concentrated cinnamon capsules taken daily, alongside acetaminophen, are where I'd be more careful, especially if you already have liver concerns. Please check with your pharmacist or doctor before combining the two.

In vitro research shows that a methanol extract of Padang cassia inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, taking Padang cassia with CYP3A4 substrates might increase the effects and side effects of these substrates.

This Colrex combo includes codeine, which is the piece worth thinking about here. Codeine is a prodrug, so your body has to activate it to relieve pain or quiet a cough. The early steps for codeine lean mostly on a different enzyme (CYP2D6), but CYP3A4 also handles part of codeine's processing. Concentrated cinnamon like Padang cassia might block CYP3A4 a little. If that happens, slightly more codeine could linger, which can mean extra drowsiness, dizziness, constipation, or slowed breathing in sensitive people.

Honestly, this is based only on lab testing, not real patients, so the risk is theoretical and likely small. Sprinkling cinnamon on food won't matter. If you take a daily cinnamon capsule, just mention it to your pharmacist or doctor before mixing.
