Acetohexamide falls under HEPATOTOXIC DRUGS — the group Padang Cassia interacts with.

There is some concern that ingesting large amounts of Padang cassia might cause hepatotoxicity in some people. Padang cassia contains coumarin. Coumarin has caused hepatotoxicity in animal models. In otherwise healthy humans, very high doses of coumarin, from 50-7000 mg daily, can result in hepatotoxicity that resolves when coumarin use is discontinued. Lower amounts might also cause liver problems in sensitive people, such as those with liver disease or those taking potentially hepatotoxic agents.

Here's the thing to know about pairing concentrated Padang cassia (a cassia-type cinnamon) with acetohexamide, an older diabetes pill in the sulfonylurea family. This cinnamon naturally contains coumarin, and in large supplement-sized amounts coumarin has been linked to liver stress. Acetohexamide itself can occasionally be hard on the liver too. So the worry is simply that stacking two things that may bother the liver could add up, especially if you already have any liver trouble.

To be clear, this is a theoretical concern based on how these things behave, not a proven reaction, and it's about big supplement doses, not cinnamon sprinkled on your oatmeal. Watch for signs like yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and check with your pharmacist or doctor before starting a cinnamon supplement.

Some clinical research shows that Padang cassia can reduce blood glucose levels in healthy patients and in patients with diabetes or impaired glucose tolerance (prediabetes). Theoretically, Padang cassia might have additive effects in patients treated with antidiabetes drugs. Dose adjustments to diabetes medications may be necessary.

Here's the thing to keep in mind: Acetohexamide is an older sulfonylurea that lowers your blood sugar by pushing your pancreas to release more insulin. Padang cassia (a cassia cinnamon) may nudge blood sugar down a little on its own. Put them together and the effects can stack, so your sugar could dip lower than expected. Watch for the usual low blood sugar signs, things like shakiness, sweating, dizziness, a racing heart, or feeling confused.

This is based on lower quality studies, so it's a possible rather than guaranteed effect. Sprinkling cinnamon on your oatmeal is unlikely to cause trouble. Concentrated cinnamon capsules are the bigger question, partly because of the blood sugar effect and partly because of coumarin and your liver. Please check with your pharmacist or doctor before adding a supplement.
