Atazanavir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Padang Cassia interacts with.

In vitro research shows that a methanol extract of Padang cassia inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, taking Padang cassia with CYP3A4 substrates might increase the effects and side effects of these substrates.

Here's the gist for atazanavir (Reyataz). Your liver uses an enzyme called CYP3A4 to clear this drug from your body. In a lab dish, an extract of Padang cassia slowed that enzyme down. If that happened in real life, atazanavir could build up a bit more than expected, which might mean stronger side effects like nausea, headache, or that yellow tint to the skin or eyes (a known atazanavir effect from raised bilirubin).

I want to be straight with you though: this is only test-tube evidence, not human studies, so it's more of a theoretical concern. Sprinkling cinnamon on your food shouldn't be an issue. The bigger question mark is concentrated cinnamon supplements. Before starting one, run it by your pharmacist or HIV provider.

Padang Cassia is a type of cinnamon (Indonesian or Korintje cinnamon) grown mainly in Indonesia and used widely as a kitchen spice. As a supplement, evidence for health benefits like blood sugar control is limited and mixed, and like other cassia cinnamons it naturally contains coumarin, which can be harmful to the liver in large amounts. Use food amounts freely, but talk to a pharmacist or doctor before taking concentrated cinnamon supplements.

About Atazanavir

What Is Atazanavir Used For?

Brand name: Reyataz

Atazanavir capsules are used together with other antiretroviral medicines to treat HIV-1 infection in adults and in children 6 years and older who weigh at least 15 kg (about 33 lbs).

Reyataz — available as capsules and as an oral powder — is also approved for HIV-1 treatment in adults and in children as young as 3 months who weigh at least 5 kg (about 11 lbs). Reyataz oral powder must always be taken with ritonavir and food, and is not for use in children weighing less than 5 kg.
