Bedaquiline falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Padang Cassia interacts with.

In vitro research shows that a methanol extract of Padang cassia inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, taking Padang cassia with CYP3A4 substrates might increase the effects and side effects of these substrates.

Here's the gist for Sirturo (bedaquiline): your body relies on a liver enzyme called CYP3A4 to clear this drug out. Lab studies suggest concentrated Padang cassia (a type of cassia cinnamon) might slow that enzyme down. If it does, bedaquiline could build up a bit more than expected, which in theory could nudge up side effects like nausea, headache, joint pain, or changes in your heart rhythm (a longer QT interval) that this TB drug is already known to cause.

I want to be honest about the strength of this, though. It's based only on test-tube research, not real people, so we don't truly know if it matters in everyday use. Sprinkling cinnamon on your food is not a concern. But because bedaquiline is a serious TB medicine, please check with your pharmacist or doctor before adding any concentrated cinnamon supplement.

Padang Cassia is a type of cinnamon (Indonesian or Korintje cinnamon) grown mainly in Indonesia and used widely as a kitchen spice. As a supplement, evidence for health benefits like blood sugar control is limited and mixed, and like other cassia cinnamons it naturally contains coumarin, which can be harmful to the liver in large amounts. Use food amounts freely, but talk to a pharmacist or doctor before taking concentrated cinnamon supplements.

What Is Bedaquiline Used For?

Brand name: Sirturo

Sirturo (bedaquiline) is used as part of a combination treatment for pulmonary TB — TB that affects the lungs — caused by bacteria resistant to at least both rifampin and isoniazid. It is approved for adults and for children 2 years and older who weigh at least 8 kg (about 17.6 lbs).

Sirturo is not used for latent TB (TB infection with no active symptoms), TB outside the lungs, drug-sensitive TB, or infections caused by non-tuberculosis mycobacteria. It must always be used alongside other TB medicines — never alone.
