Ibrutinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Padang Cassia interacts with.

In vitro research shows that a methanol extract of Padang cassia inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, taking Padang cassia with CYP3A4 substrates might increase the effects and side effects of these substrates.

Here's the gist for Imbruvica (ibrutinib): your body relies heavily on an enzyme called CYP3A4 to break this drug down and keep its levels in check. In lab dishes, a concentrated extract of Padang cassia slowed that same enzyme down. If that happened in a real person, ibrutinib could clear out more slowly and pile up higher than intended.

Ibrutinib already carries a real risk of bruising and bleeding, plus diarrhea, irregular heartbeat, and infections, and higher levels could nudge those side effects up. The good news: this is only test-tube evidence, not proof it happens in people. Sprinkling cinnamon on your food is no concern. But before using a concentrated cinnamon supplement, please check with your oncologist or pharmacist first.

Padang Cassia is a type of cinnamon (Indonesian or Korintje cinnamon) grown mainly in Indonesia and used widely as a kitchen spice. As a supplement, evidence for health benefits like blood sugar control is limited and mixed, and like other cassia cinnamons it naturally contains coumarin, which can be harmful to the liver in large amounts. Use food amounts freely, but talk to a pharmacist or doctor before taking concentrated cinnamon supplements.

What Is Ibrutinib Used For?

Brand name: Imbruvica

Imbruvica (ibrutinib) is used in adults to treat chronic lymphocytic leukemia (CLL) and small lymphocytic lymphoma (SLL), including cases with a specific genetic change called 17p deletion. It is also used in adults to treat Waldenström's macroglobulinemia (WM), a rare type of slow-growing blood cancer.

Imbruvica is also approved for adults and children aged 1 year and older with chronic graft-versus-host disease (cGVHD) — a serious immune complication that can occur after a stem cell or bone marrow transplant — when at least one prior treatment has not worked.
