Here's the thing to know about combining concentrated cinnamon supplements with Opsumit (macitentan). This isn't about cinnamon changing how your body breaks the drug down. Instead, both can put a little extra stress on the liver. Macitentan can occasionally raise liver enzymes on its own, and cassia cinnamon (including Padang cassia) naturally contains coumarin, which in large concentrated doses has been linked to liver irritation.
To be clear, this concern is theoretical and based mostly on how these things behave, not on solid human studies. The sprinkle of cinnamon in your oatmeal or coffee is not a problem. It's the high-dose capsules that could add up. If you're tempted to take a cinnamon supplement, please check with your pharmacist or doctor first so your liver labs can be watched.

Here's the quick version: your body uses an enzyme called CYP3A4 to clear macitentan (Opsumit) out of your system. In test-tube studies, a concentrated extract of Padang cassia slowed that same enzyme down. If that held true in people, macitentan could build up a bit more than expected, which in theory might mean stronger effects and more side effects like headache, low blood pressure, swelling in your legs, a stuffy nose, or feeling more run-down.
The important caveat: this is based only on lab research, not real patient cases, so it's a maybe and not a proven problem. Sprinkling cinnamon on your food is very unlikely to matter. If you're considering a concentrated cinnamon supplement, just check with your pharmacist or doctor first.

What Is Macitentan Used For?
Brand name: Opsumit
Macitentan tablets — sold under the brand name Opsumit — are used to treat pulmonary arterial hypertension (PAH, WHO Group I), a serious condition where the blood pressure in the arteries supplying the lungs is abnormally high. The goal is to slow down how quickly the disease gets worse and to lower the chances of being hospitalized because of it.
In clinical studies, it was shown to be effective in adults with mostly moderate-to-severe PAH symptoms. Patients in those studies had PAH caused by unknown or inherited reasons, connective tissue disorders, or congenital heart disease with repaired heart shunts.
