Why this applies to Pazopanib

Pazopanib falls under HEPATOTOXIC DRUGS — the group Padang Cassia interacts with.

There is some concern that ingesting large amounts of Padang cassia might cause hepatotoxicity in some people. Padang cassia contains coumarin. Coumarin has caused hepatotoxicity in animal models. In otherwise healthy humans, very high doses of coumarin, from 50-7000 mg daily, can result in hepatotoxicity that resolves when coumarin use is discontinued. Lower amounts might also cause liver problems in sensitive people, such as those with liver disease or those taking potentially hepatotoxic agents.

Here's the thing with pazopanib (Votrient): it's already known to be tough on the liver. The drug label even carries a warning about liver injury, which is why your oncologist checks your liver blood work regularly while you're on it. So the worry here isn't about cinnamon changing how pazopanib works in your body. It's that you'd be stacking two things that can each stress the liver.

Concentrated cassia cinnamon supplements (like Padang cassia) contain coumarin, which in large amounts has been linked to liver trouble. Piling that on top of pazopanib could, in theory, add to the strain and show up as yellowing skin or eyes, dark urine, belly pain, or fatigue. This is a theoretical concern, not proven, and a sprinkle of cinnamon on your food is not the issue. Concentrated capsules are. Please run any cinnamon supplement past your oncology team first.

Pazopanib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

In vitro research shows that a methanol extract of Padang cassia inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, taking Padang cassia with CYP3A4 substrates might increase the effects and side effects of these substrates.

Here's the gist for Pazopanib (Votrient): your liver clears it using an enzyme called CYP3A4. In lab dish studies, a concentrated extract of Padang cassia slowed that same enzyme down. If that were to happen in a person, less pazopanib would get broken down, so more could build up in your body and push its effects (and side effects) higher.

With pazopanib, that could mean more high blood pressure, fatigue, diarrhea, nausea, or liver strain. I want to be honest though, this is only a theoretical concern from test-tube research, not proven in people. Sprinkling cinnamon on your food is very unlikely to matter. Before starting a concentrated cinnamon supplement, please run it by your oncologist or pharmacist first.

Padang Cassia is a type of cinnamon (Indonesian or Korintje cinnamon) grown mainly in Indonesia and used widely as a kitchen spice. As a supplement, evidence for health benefits like blood sugar control is limited and mixed, and like other cassia cinnamons it naturally contains coumarin, which can be harmful to the liver in large amounts. Use food amounts freely, but talk to a pharmacist or doctor before taking concentrated cinnamon supplements.

What Is Pazopanib Used For?

Brand name: Votrient

Pazopanib tablets — sold as the brand Votrient and as generic pazopanib — are used to treat two types of advanced cancer in adults: advanced renal cell carcinoma (RCC), the most common form of kidney cancer, and advanced soft tissue sarcoma (STS) in patients who have already received prior chemotherapy.

Important limitation: Pazopanib has not been shown to work in adipocytic soft tissue sarcoma (a subtype involving fat cells) or in gastrointestinal stromal tumors (GISTs), so it is not used for those cancers.
