Padang Cassia and Pramlintide

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ANTIDIABETES DRUGS

Why this applies to Pramlintide

Pramlintide falls under ANTIDIABETES DRUGS — the group Padang Cassia interacts with.

Some clinical research shows that Padang cassia can reduce blood glucose levels in healthy patients and in patients with diabetes or impaired glucose tolerance (prediabetes). Theoretically, Padang cassia might have additive effects in patients treated with antidiabetes drugs. Dose adjustments to diabetes medications may be necessary.
Severity Moderate Likelihood Possible Evidence B

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to keep on your radar: pramlintide already works hard to bring down your after-meal blood sugar by slowing how fast your stomach empties and trimming your appetite. Some lower-quality studies suggest that Padang cassia cinnamon may nudge blood sugar down a bit on its own. Stack the two together and the effects could add up, so your sugar might drift lower than you expect.

The realistic worry here is hypoglycemia, shakiness, sweating, dizziness, or feeling confused, especially since you're also using mealtime insulin. The evidence for cinnamon is modest, and a sprinkle on your oatmeal is unlikely to matter. Concentrated cinnamon capsules are a different story. Before starting any, check in with your pharmacist or doctor so your doses can be watched.

This page covers Padang Cassia only. If you take other supplements — a multivitamin, a sleep aid, something for joints — the Pramlintide page lists 239 supplements it has a documented interaction with, worst first, each one explained.

The most serious there right now: Siberian Cocklebur (major), Acai (moderate), African Wild Potato (moderate), Agaricus Mushroom (moderate) and Agmatine (moderate).

By severity: 1 major · 208 moderate · 30 minor.

From our pharmacist’s overview on the Pramlintide page

“The single entry rated major on the pramlintide (Symlin) list is Siberian cocklebur, which has caused severe hypoglycemia in humans, not just in lab studies. That matters more than usual here because pramlintide is injected at mealtimes alongside insulin, and a dangerous low is already the main hazard of that pairing.”

About Padang Cassia

What is Padang Cassia?

  • Adding flavor to food
  • Digestive upset
  • Blood sugar support
  • Antimicrobial purposes
  • Antioxidant support

Padang Cassia is a type of cinnamon (Indonesian or Korintje cinnamon) grown mainly in Indonesia and used widely as a kitchen spice. As a supplement, evidence for health benefits like blood sugar control is limited and mixed, and like other cassia cinnamons it naturally contains coumarin, which can be harmful to the liver in large amounts. Use food amounts freely, but talk to a pharmacist or doctor before taking concentrated cinnamon supplements.

About Pramlintide Injection

What Is Pramlintide Injection Used For?

Brand name: Symlin

SymlinPen (pramlintide) is used as an add-on treatment for adults with type 1 or type 2 diabetes who already use mealtime insulin but still haven't reached their target blood sugar levels — even with optimal insulin therapy.

It is not a replacement for insulin. It works alongside insulin, given right before major meals, to reduce the spike in blood sugar that happens after eating.
