Selumetinib Sulfate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Padang Cassia interacts with.

In vitro research shows that a methanol extract of Padang cassia inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, taking Padang cassia with CYP3A4 substrates might increase the effects and side effects of these substrates.

Here's the gist for Koselugo (selumetinib). Your child's body clears this medicine using a liver enzyme called CYP3A4. In a lab dish, an extract of Padang cassia slowed that same enzyme down. The worry, in theory, is that a concentrated cinnamon supplement could let selumetinib build up higher than intended, which may mean more of its known side effects: nausea, vomiting, diarrhea, mouth sores, skin rash, fatigue, or changes in heart function.

I want to keep this in perspective. This is only test-tube evidence, not proof it happens in real children, and sprinkling cinnamon on toast or oatmeal is very unlikely to matter. Concentrated cinnamon capsules are a different story. Please run any cinnamon supplement past your child's oncology team or pharmacist before starting it.

Padang Cassia is a type of cinnamon (Indonesian or Korintje cinnamon) grown mainly in Indonesia and used widely as a kitchen spice. As a supplement, evidence for health benefits like blood sugar control is limited and mixed, and like other cassia cinnamons it naturally contains coumarin, which can be harmful to the liver in large amounts. Brand name: Koselugo

Selumetinib, sold as Koselugo, treats neurofibromatosis type 1 (NF1) in adults and children 1 year of age and older. It is for people whose plexiform neurofibromas (nerve sheath tumors) cause symptoms and cannot be removed by surgery.

Koselugo comes as capsules and as oral granules. Both are used for this same condition.
