Sotorasib falls under HEPATOTOXIC DRUGS — the group Padang Cassia interacts with.

There is some concern that ingesting large amounts of Padang cassia might cause hepatotoxicity in some people. Padang cassia contains coumarin. Coumarin has caused hepatotoxicity in animal models. In otherwise healthy humans, very high doses of coumarin, from 50-7000 mg daily, can result in hepatotoxicity that resolves when coumarin use is discontinued. Lower amounts might also cause liver problems in sensitive people, such as those with liver disease or those taking potentially hepatotoxic agents.

Here's the concern with combining concentrated Padang cassia (a cassia-type cinnamon) and Sotorasib. Both can be tough on the liver. Cassia cinnamon naturally contains coumarin, which in large supplement doses has been linked to liver stress. Sotorasib itself is already known to raise liver enzymes and can cause liver injury in some people, so doctors usually monitor your bloodwork while you're on it. Stacking a coumarin-heavy supplement on top could add to that burden.

I want to be honest: this is a theoretical caution based on how these things behave, not proven cases. Sprinkling cinnamon on your food is not the issue. Concentrated cinnamon capsules are. Please run any supplement past your oncology team or pharmacist first.

Sotorasib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

In vitro research shows that a methanol extract of Padang cassia inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, taking Padang cassia with CYP3A4 substrates might increase the effects and side effects of these substrates.

Here's the short version for Lumakras (sotorasib): your liver uses an enzyme called CYP3A4 to help clear this cancer drug from your body. In lab dish studies, a concentrated extract of Padang cassia cinnamon seemed to slow that enzyme down. If that held true in people, sotorasib could build up a bit higher than expected, which in theory might mean more of its usual side effects like nausea, diarrhea, fatigue, or changes in liver blood tests.

Important to keep in mind: this is only test-tube evidence, not proof it happens in real patients, so I wouldn't panic. Sprinkling cinnamon on your food is unlikely to matter. The bigger question is concentrated cinnamon capsules. With any cancer therapy, please run supplements past your oncology team or pharmacist first.

Padang Cassia is a type of cinnamon (Indonesian or Korintje cinnamon) grown mainly in Indonesia and used widely as a kitchen spice. As a supplement, evidence for health benefits like blood sugar control is limited and mixed, and like other cassia cinnamons it naturally contains coumarin, which can be harmful to the liver in large amounts. Use food amounts freely, but talk to a pharmacist or doctor before taking concentrated cinnamon supplements.

What Is Sotorasib Used For?

Brand name: Lumakras

Lumakras is used to treat adults with KRAS G12C-mutated locally advanced or metastatic non-small cell lung cancer (NSCLC) who have already received at least one prior cancer treatment. This use was approved under the FDA's accelerated approval pathway based on tumor response rates, and continued approval may depend on results from confirmatory trials.

Lumakras is also used in combination with panitumumab to treat adults with KRAS G12C-mutated metastatic colorectal cancer (mCRC) who have previously received certain chemotherapy regimens. In both cases, patients must first be confirmed to carry the KRAS G12C mutation using an FDA-approved test.
