Tocilizumab falls under HEPATOTOXIC DRUGS — the group Padang Cassia interacts with.

There is some concern that ingesting large amounts of Padang cassia might cause hepatotoxicity in some people. Padang cassia contains coumarin. Coumarin has caused hepatotoxicity in animal models. In otherwise healthy humans, very high doses of coumarin, from 50-7000 mg daily, can result in hepatotoxicity that resolves when coumarin use is discontinued. Lower amounts might also cause liver problems in sensitive people, such as those with liver disease or those taking potentially hepatotoxic agents.

Here's the simple version: this isn't about cinnamon changing how Tocilizumab (Actemra) works in your body. It's about a shared stress on your liver. Tocilizumab can sometimes raise liver enzymes and, rarely, cause liver injury, which is why your doctor checks bloodwork while you're on it. Padang cassia (a cassia-type cinnamon) naturally contains coumarin, which in large, concentrated amounts has been linked to liver irritation in some people.

So the concern is stacking two things that can each lean on the liver. Honestly, this is theoretical and based mostly on the pharmacology, not strong human studies. The pinch you sprinkle on oatmeal is almost certainly fine. Daily concentrated cinnamon capsules are the real question. Please run those by your pharmacist or doctor before starting.

Padang Cassia is a type of cinnamon (Indonesian or Korintje cinnamon) grown mainly in Indonesia and used widely as a kitchen spice. What Is Tocilizumab Injection Used For?

Brand name: Actemra

Tocilizumab injection is used to treat several serious inflammatory and immune-related conditions. Actemra (tocilizumab) is indicated for moderately to severely active rheumatoid arthritis in adults who haven't responded well to other disease-modifying drugs, giant cell arteritis in adults, systemic sclerosis-associated interstitial lung disease (SSc-ILD) to slow lung function decline, polyarticular and systemic juvenile idiopathic arthritis in patients 2 years and older, and severe or life-threatening cytokine release syndrome (CRS) caused by CAR T-cell therapy in adults and children 2 and older.

Actemra is also approved for hospitalized adults and children aged 2 and older with COVID-19 who are on corticosteroids and need supplemental oxygen or mechanical breathing support.
