Panax Notoginseng and Aspirin, Caffeine

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Panax Notoginseng interacts with whole categories of medication. Aspirin, Caffeine falls into 3 of those categories, so it can interact with Panax Notoginseng in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

1 of 3

Why this applies to Aspirin, Caffeine

Aspirin, Caffeine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Panax Notoginseng interacts with.

Theoretically, taking Panax notoginseng might reduce the levels and clinical effects of CYP1A2 substrates.
Animal research shows that administering Panax notoginseng intravenously for 7 days before intraperitoneal injection of caffeine can decrease maximal blood levels of caffeine by 37%. This interaction was attributed to the ability of Panax notoginseng to increase the activity of CYP1A2.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Here's the gist with this combo. The caffeine portion of your Anacin is cleared from your body by a liver enzyme called CYP1A2. Panax notoginseng may speed that enzyme up, which could clear caffeine faster and leave a bit less of it around. In plain terms, you might notice the caffeine's pick-me-up feels weaker, less alertness or wakefulness than you'd expect.

Honestly, though, this is based only on animal lab data, not on people, so I wouldn't lose sleep over it. There's a separate point worth mentioning: notoginseng is traditionally used for bleeding and may thin the blood a little, and aspirin already does that, so together they could raise bruising or bleeding risk. Run it by your pharmacist or doctor before combining them.

ASPIRIN

2 of 3
Theoretically, taking Panax notoginseng concomitantly with aspirin may increase the risk of adverse effects from both products.
Animal research shows that taking Panax notoginseng extract with aspirin increases blood levels of salicylic acid by approximately 50% and blood levels of Panax notoginseng by 75% to 196%. This effect may be due to increased absorption of both products.

Dr. Brian Staiger, PharmD, BCPS

Your Anacin combines aspirin with a little caffeine, and it's the aspirin part that matters here. In animal studies, pairing Panax notoginseng with aspirin pushed blood levels of salicylic acid (aspirin's active form) up by roughly 50%, likely from better absorption of both. More aspirin floating around can mean a higher chance of stomach upset, heartburn, or, more importantly, bleeding and bruising. Since this herb is also traditionally used to affect blood clotting, the two together could nudge that risk up further.

Keep in mind this is theoretical and based on lab and animal work, not strong human data, so don't panic. Still, before combining them, please check with your pharmacist or doctor first.

Here's what I'd want you to know about pairing this herb with a product like Anacin, which combines aspirin and caffeine. The interaction on file is really about the caffeine part. Panax notoginseng may speed up an enzyme in your liver called CYP1A2, the same one that clears caffeine out of your system. If caffeine gets cleared faster, you may notice less of its usual pick-me-up, so the alertness or headache relief you expect from it could feel weaker.

I'll be honest with you though: this comes from animal research only, where caffeine levels dropped about 37%. We don't have solid human data, so it's more of a theoretical concern than a proven problem. The aspirin part isn't what this warning is about. Still, since both this herb and aspirin can affect bleeding, it's worth a quick chat with your pharmacist or doctor before combining them.

From our pharmacist’s overview on the Aspirin, Caffeine page

“Anacin combines aspirin with caffeine, so supplements can hit it from two directions, and a few of the 347 on this list deserve real respect. The biggest group, over 200 ingredients, can raise bleeding risk on top of aspirin's blood-thinning effect; beer and wine carry major ratings because alcohol plus an aspirin-type drug makes stoma…”
