Atazanavir, Cobicistat falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Passion Flower interacts with.
In vitro research suggests that passion flower can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for your Evotaz (atazanavir plus cobicistat). Lab studies suggest passion flower might gently speed up an enzyme called CYP3A4, the same pathway your HIV medicine relies on. In theory, that could nudge your body to clear the atazanavir a little faster, which might mean slightly less drug working to keep your virus in check. With HIV treatment, keeping blood levels steady really matters, so anything that lowers them is worth taking seriously.
That said, this is very weak evidence (only test-tube data), and the effect looked much smaller than known strong inducers. I wouldn't panic, but I also wouldn't add passion flower on your own. Please check with your pharmacist or HIV provider first so they can keep your treatment on track.
