Why this applies to Irinotecan Hydrochloride

Irinotecan Hydrochloride falls under ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP) — the group Passion Flower interacts with.

Theoretically, passion flower might reduce the bioavailability of OATP2B1 and OATP1A2 substrates.
In vitro research shows that the passion flower constituents apigenin and vitexin inhibit OATP2B1 and OATP1A2. This inhibition may be dose-dependent. One specific high-flavonoid passion flower extract (Valverde) seems to inhibit OATP2B1 and OATP1A2, while another extract with a lower flavonoid concentration (Arkocaps) shows less potent inhibition. OATPs are responsible for the uptake of drugs and other compounds into the body; however, the specific activities of OATP2B1 and OATP1A2 are not well characterized.

Here's the honest picture with passion flower and your irinotecan (Onivyde). The concern on file is that certain passion flower compounds (like apigenin and vitexin) might block transporters called OATPs, which help shuttle some drugs into cells. In theory, that could slightly change how much irinotecan your body takes up. But I want to be straight with you: this is rated a minor interaction based only on lab dish studies, not real people, so we don't truly know if it matters in everyday life.

Because irinotecan is a serious chemotherapy drug where small shifts can affect both how well it works and side effects like diarrhea and low blood counts, I'd play it safe. Please loop in your oncology team or pharmacist before adding passion flower rather than starting it on your own.

Theoretically, passion flower might decrease the effects of CYP3A4 substrates.
In vitro research suggests that passion flower can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.

Here's the short version on combining passion flower with irinotecan (Onivyde). Your liver uses an enzyme called CYP3A4 to help process irinotecan. Some lab and animal research hints that passion flower might gently speed up that enzyme, which in theory could clear the drug a bit faster and slightly weaken how well it works against the cancer. I'll be honest though: this is very weak, test-tube level evidence, and the effect was far smaller than known strong inducers like rifampin.

Because irinotecan is a serious chemotherapy drug, I wouldn't add passion flower (or really any supplement) without first running it by your oncology team. They keep a close eye on your treatment, and they can tell you what's safe to take alongside it.
