Fosnetupitant, Palonosetron (IV) falls under SEROTONERGIC DRUGS — the group Psilocybin interacts with.

Theoretically, combining psilocybin with serotonergic drugs might cause additive serotonergic effects.
Psilocybin has serotonin 2A (5-HT2A) receptor agonist activity. Theoretically, combining serotonergic drugs with psilocybin might increase the risk of serotonergic side effects, including serotonin syndrome. However, this interaction has not been reported in humans.

Here's the thinking behind this one. The palonosetron part of Akynzeo works on serotonin (it blocks the 5-HT3 receptor to calm nausea), while psilocybin turns into psilocin and stirs up serotonin activity at a different receptor (5-HT2A). When two things both nudge the serotonin system, there's a theoretical worry about too much serotonin, called serotonin syndrome. That can show up as agitation, a racing heart, sweating, shivering, tremor, or confusion.

I want to be honest, though: this is based on how the drugs work on paper, not on real cases in people. It hasn't actually been reported. Still, psilocybin should only ever be used under medical supervision, and your cancer care team should know about anything you take. Please loop in your oncologist or pharmacist before mixing the two.

What Is Netupitant and Palonosetron Used For?

Brand name: Akynzeo

Netupitant and palonosetron helps prevent nausea and vomiting from cancer chemotherapy in adults. It is used together with dexamethasone, a steroid medicine. It covers both the first day (acute phase) and the days after (delayed phase).

Akynzeo capsules are used for initial and repeat courses of chemotherapy, including highly emetogenic (strongly nausea-causing) chemotherapy. Akynzeo for injection and Akynzeo injection are used for highly emetogenic chemotherapy only. They have not been studied with anthracycline plus cyclophosphamide chemotherapy.
