Theoretically, combining psilocybin with serotonergic drugs might cause additive serotonergic effects.
Psilocybin has serotonin 2A (5-HT2A) receptor agonist activity. Theoretically, combining serotonergic drugs with psilocybin might increase the risk of serotonergic side effects, including serotonin syndrome. However, this interaction has not been reported in humans.

Here's the thing about tramadol that makes this pairing worth a second thought: besides easing pain, it nudges up serotonin levels in your brain. Psilocybin works on serotonin too, by switching on certain receptors. Stack the two and you could, in theory, get too much serotonin activity at once.

That's what we call serotonin syndrome, and the warning signs include agitation, a racing heart, sweating, shaking, muscle twitching, or feeling dizzy and confused. I want to be honest with you though: this is a theoretical concern based on how both work, and it hasn't actually been reported in people taking them together. Still, tramadol carries its own seizure risk, so don't combine these without talking to your doctor or pharmacist first.
