SEROTONERGIC DRUGS
Why this applies to Meperidine
Meperidine falls under SEROTONERGIC DRUGS — the group Psilocybin interacts with.
Psilocybin has serotonin 2A (5-HT2A) receptor agonist activity. Theoretically, combining serotonergic drugs with psilocybin might increase the risk of serotonergic side effects, including serotonin syndrome. However, this interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the concern with mixing these two. Meperidine doesn't just relieve pain like a typical opioid. It also nudges serotonin levels upward in the brain, which makes it stand out from many other pain medications. Psilocybin works on serotonin too, mainly by switching on certain serotonin receptors. Stack them together and, in theory, you could pile on too much serotonin activity at once.
When serotonin gets too high, people can develop what's called serotonin syndrome: agitation, a racing heart, sweating, shivering, muscle twitching, or confusion. To be fair, this is a theoretical worry based on how the drugs work, and it hasn't actually been reported in real people with this exact pair. Still, meperidine is a well-known serotonin offender, so please run any combination past your pharmacist or doctor first.

About Meperidine
What Is Meperidine Used For?
Brand names: Pethadol, Cytospaz
Meperidine is used when pain is severe enough to need an opioid and other treatments have not worked or are not suitable.
Tablets and oral solution are used for acute (short-term) pain. They should not be used for chronic pain.
Meperidine Hydrochloride Injection and Demerol Injection are also used for preoperative medication (given before surgery), support of anesthesia, and obstetrical analgesia (pain relief during childbirth).
