Pu-erh Tea and Aspirin, Butalbital, Caffeine, Codeine Phosphate

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Pu-erh Tea interacts with whole categories of medication. Aspirin, Butalbital, Caffeine, Codeine Phosphate falls into 2 of those categories, so it can interact with Pu-erh Tea in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

ANTICOAGULANT/ANTIPLATELET DRUGS

Why this applies to Aspirin, Butalbital, Caffeine, Codeine Phosphate

Aspirin, Butalbital, Caffeine, Codeine Phosphate falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Pu-erh Tea interacts with.

Theoretically, pu-erh tea may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
Pu-erh tea contains caffeine. Caffeine is reported to have antiplatelet activity. Theoretically, the caffeine in pu-erh tea might increase the risk of bleeding when used concomitantly with antiplatelet drugs. However, this interaction has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to know about this combination: your medication already contains aspirin, which is a blood thinner that makes platelets less sticky. Pu-erh tea brings two things to the table that point in the same direction, caffeine (which has been reported in lab studies to have a mild antiplatelet effect) and tea polyphenols. So in theory, drinking a lot of pu-erh alongside this aspirin-containing product could nudge your bleeding risk a little higher, meaning easier bruising, nosebleeds, or stomach upset and bleeding.

That said, this is a theoretical concern based only on lab and animal data, and it hasn't actually been shown in people. A normal cup or two as a daily drink is unlikely to cause trouble. I'd just mention your tea habit to your pharmacist or doctor, especially if you notice unusual bruising or bleeding.

STIMULANT DRUGS

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: your Fiorinal with Codeine already has caffeine built right into each tablet. Pu-erh tea adds even more caffeine on top of that. When you stack the two, you may notice the typical caffeine overload feelings, things like jitteriness, a racing or pounding heart, trouble sleeping, an upset stomach, or feeling wired and anxious.

This is more of a common-sense, expert-opinion concern than a dangerous reaction, and an occasional cup with your medication isn't likely to cause real trouble for most people. Still, if you drink pu-erh regularly or in large amounts, it adds up. If you feel overly stimulated, mention it to your pharmacist or doctor so they can help you find a comfortable balance.

From our pharmacist’s overview on the Aspirin, Butalbital, Caffeine, Codeine Phosphate page

“Fiorinal with Codeine packs four active drugs into one capsule, and I would treat its supplement list with genuine wariness rather than a quick skim.
