Red yeast rice contains varying levels of the statin drug lovastatin, which is metabolized by CYP3A4. Combining red yeast rice with CYP3A4 inhibitors might increase serum levels of lovastatin from red yeast rice.

Here's the thing to know about Cobicistat (Tybost): its whole job is to block a liver enzyme called CYP3A4 so your HIV medicine sticks around longer. Red yeast rice contains lovastatin, a statin that the same CYP3A4 enzyme normally clears out. So when cobicistat slows that enzyme down, the lovastatin from your red yeast rice can build up higher than expected.

More statin in your blood means a higher chance of statin-type side effects, mainly muscle aches, weakness, and rarely, more serious muscle or liver problems. This is based on how the drugs work (a theoretical concern) rather than firm studies, and red yeast rice doses vary a lot. Still, please check with your pharmacist or doctor before combining them, since cobicistat is a strong enzyme blocker.

Red yeast rice contains monacolin K, which is chemically identical to the prescription statin lovastatin, and it can lower LDL ('bad') cholesterol. What Is Cobicistat Used For?

Brand name: Tybost

Tybost (cobicistat) is not itself an HIV treatment — it works by slowing the breakdown of certain HIV medicines so they stay active in your body long enough to work. Specifically, Tybost is used to boost the HIV medicines atazanavir or darunavir (once-daily dosing) as part of a complete HIV-1 treatment regimen in adults and in children weighing at least 14 kg (about 31 lbs).

Tybost is not interchangeable with ritonavir (another booster) for all HIV medicines. It is not recommended with darunavir twice-daily, fosamprenavir, saquinavir, or tipranavir.
