Felbamate falls under HEPATOTOXIC DRUGS — the group Red Yeast Rice interacts with.

Theoretically, concomitant use might increase the risk of liver damage.
Red yeast rice contains varying levels of the drug lovastatin. Lovastatin can cause liver damage in some people. Some clinical research suggests that supplements containing red yeast rice might increase liver enzyme levels in some, but not all, participants. Cases of acute hepatitis have been associated with red yeast rice. Combining it with hepatotoxic drugs might further increase this risk.

Here's the concern with pairing red yeast rice and felbamate. Felbamate is already one of those seizure medicines we watch closely for the liver, since it carries a known risk of liver injury (your doctor likely checks liver bloodwork while you're on it). Red yeast rice naturally contains lovastatin, a statin that can also stress the liver in some people. Stacking two things that can each be hard on the liver may add up.

To be fair, this is a theoretical caution based on how these substances behave, not a proven, common problem, so I don't want you to panic. It doesn't change how well your felbamate controls seizures. Still, watch for yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and check with your pharmacist or doctor before combining them.

Red yeast rice contains monacolin K, which is chemically identical to the prescription statin lovastatin, and it can lower LDL ('bad') cholesterol. Because of this, it carries the same risks as statin drugs, and the amount of active ingredient varies widely between products. About Felbamate

What Is Felbamate Used For?

Brand name: Felbatol

Felbamate is not a first-line seizure medication. It is used only when a person's epilepsy has not responded well enough to other treatments and the seizures are severe enough that the serious risks of this drug are considered acceptable.

In adults, felbamate can be used alone or alongside other antiepileptic drugs (AEDs) to treat partial seizures (seizures that start in one part of the brain), with or without spreading to become a full seizure. In children, felbamate is used as an add-on treatment for both partial and generalized seizures linked to Lennox-Gastaut syndrome, a severe form of childhood epilepsy. Felbatol is specifically labeled for these uses in both adults and children.
