Here's the thing to know about pairing red yeast rice with Fosaprepitant (Emend Injection). Red yeast rice naturally contains lovastatin, a statin that your liver clears using an enzyme called CYP3A4. Fosaprepitant can gently slow that same enzyme down. When it does, the lovastatin from your red yeast rice may hang around longer than usual and build up a bit higher than expected.

Higher statin levels can mean a greater chance of muscle aches, weakness, or, rarely, more serious muscle or liver trouble. I'll be honest with you though: this is a theoretical concern based on how the drugs work, not something strongly proven, and Emend is usually given for just a short stretch around chemotherapy. Still, please mention your red yeast rice to your pharmacist or doctor so they can keep an eye on you.
