Red Yeast Rice and Imatinib

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS

Why this applies to Imatinib

Imatinib falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Red Yeast Rice interacts with.

Theoretically, drugs that inhibit the CYP3A4 enzymes might increase levels of lovastatin from red yeast rice.
Red yeast rice contains varying levels of the statin drug lovastatin, which is metabolized by CYP3A4. Combining red yeast rice with CYP3A4 inhibitors might increase serum levels of lovastatin from red yeast rice.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to know about pairing red yeast rice with imatinib (Gleevec). Red yeast rice naturally contains lovastatin, a statin that your liver clears using an enzyme called CYP3A4. Imatinib happens to slow that same enzyme down. So in theory, taking them together could cause the lovastatin from your red yeast rice to build up higher than expected, which raises the odds of statin-type side effects like muscle aches, muscle weakness, or (rarely) more serious muscle breakdown and liver irritation.

I'll be honest, this is a theoretical concern based on how these drugs work, not something proven in studies. Still, since you're on a cancer medicine, please loop in your oncologist or pharmacist before adding red yeast rice rather than starting it on your own.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

From our pharmacist’s overview on the Imatinib page

“Imatinib (Gleevec) treats certain leukemias and gastrointestinal tumors, and because keeping its blood level steady is what keeps the cancer controlled, this list deserves genuine attention before you add anything to it. Most of the interactions trace back to how the body clears imatinib through an enzyme called CYP3A4.”

About Red Yeast Rice

What is Red Yeast Rice?

Red Yeast Rice
  • High cholesterol
  • Heart health
  • Metabolic syndrome
  • Statin alternative

Red yeast rice contains monacolin K, which is chemically identical to the prescription statin lovastatin, and it can lower LDL ('bad') cholesterol. Because of this, it carries the same risks as statin drugs, and the amount of active ingredient varies widely between products. Always talk to your pharmacist or doctor before using it, especially if you have liver problems or take other medicines.
